Your contribution will be attributed to you if you include your full name in the boxes provided above. This gift will be deposited into a non-interest bearing gift account after receipt by the Future Path 529 Plan in good order and then transferred directly into the beneficiary's Future Path 529 Plan account approximately five business days later.

Accounts in the Future Path 529 Plan are controlled by the Account Owner. Your gift contribution may have gift tax consequences or other tax implications. Please consult your tax advisor.

If you or the beneficiary are a resident of a state other than Nevada, before you make a contribution you should check with your home state to see if it offers a 529 program with state tax or other benefits that are only available for investments in such state's qualified tuition program.
